SII is a calculated index that combines platelet, neutrophil, and lymphocyte counts. Researchers use SII to investigate relationships between circulating blood-cell populations and systemic immune-inflammatory activity.

The science, plainly
Understanding SII & SIRI
SII = Platelets × Neutrophils ÷ Lymphocytes
Platelets and neutrophils sit in the numerator; lymphocytes in the denominator. The index rises when platelet or neutrophil counts are relatively high compared with lymphocytes.
SIRI = Neutrophils × Monocytes ÷ Lymphocytes
SIRI swaps platelets for monocytes, weighting the innate-cell side of the count differently. The two indices are related but not interchangeable.
A complete blood count is already collected in routine care, so indices built from it are inexpensive and widely available. Combining several cell populations into one number is one way researchers summarise the balance between them, and that composite has been examined across a broad range of study populations. It remains a research measure, not a clinical test.
No single calculated index is designed to diagnose a condition on its own. SII and SIRI are research measures meant to be interpreted alongside your broader health picture, other lab results, and guidance from a qualified healthcare professional.
A single blood draw reflects one moment in time. Tracking SII and SIRI across multiple CBCs can offer more meaningful context than any one result viewed in isolation.
